Job Description
Join Christian Living Communities as an Entry-Level MDS Coordinator, where you'll collaborate with various departments to develop and manage resident assessments and care plans. This role offers on-the-job training for LPNs and the chance to make a meaningful impact in residents' lives.
Key Responsibilities
Oversee and evaluate gathering of information for assessments and care plans
Monitor and guide completion of PPS, OBRA assessments, and CAAs
Maintain and update policies and procedures for resident assessment and care plan
Transmit completed MDS to appropriate parties weekly
Assess prospective residents with team members
Perform nursing functions when needed
Complete administrative duties such as medical forms and reports
Supervise hiring, performance management, and evaluations
Take on call status as assigned on a rotating schedule
Develop and maintain quality assurance program for assessments and care plans
Participate in on-site surveys by authorized government agencies
Other duties as assigned
Required Qualifications
Colorado State Licensed Practical Nurse license, in good standing
Minimum of two (2) years of managerial experience in a hospital or nursing care
Knowledge and skills necessary to provide care appropriate to age-related needs
Schedule flexibility
Ability to solve problems independently
Basic knowledge of office software applications (Word, Outlook, Excel)
Ability to read, write and speak English
Excellent customer service skills
