Job Description

Join Danbury as a Memory Care Coordinator and make a meaningful impact on the lives of residents while working with a supportive family-oriented team. You'll plan and coordinate programs that cater to the emotional, social, and physical needs of residents, ensuring a high standard of care and community engagement.

Key Responsibilities

  • Plan and coordinate therapeutic programs for residents
  • Assess resident characteristics and organize program content accordingly
  • Monitor daily functioning of the memory care neighborhood
  • Assist in developing and conducting training for staff on memory care programs
  • Market the program through involvement in community organizations
  • Maintain accurate documentation in compliance with regulations
  • Assist with pre-admission assessments and resident transitions

Required Qualifications

  • Must be an LPN
  • Two years of previous experience in programming including plan of care programs and coordinating meetings with POAs and families

Preferred Qualifications

  • Experience with Alzheimer’s and other dementia individuals
  • 3-5 years of experience in memory care
  • Background in nursing providing one-on-one care for seniors
  • Flexible schedule including availability to work evenings, weekends, and holidays
