Job Description
Join Danbury as a Memory Care Coordinator and make a meaningful impact on the lives of residents while working with a supportive family-oriented team. You'll plan and coordinate programs that cater to the emotional, social, and physical needs of residents, ensuring a high standard of care and community engagement.
Key Responsibilities
Plan and coordinate therapeutic programs for residents
Assess resident characteristics and organize program content accordingly
Monitor daily functioning of the memory care neighborhood
Assist in developing and conducting training for staff on memory care programs
Market the program through involvement in community organizations
Maintain accurate documentation in compliance with regulations
Assist with pre-admission assessments and resident transitions
Required Qualifications
Must be an LPN
Two years of previous experience in programming including plan of care programs and coordinating meetings with POAs and families
Preferred Qualifications
Experience with Alzheimer’s and other dementia individuals
3-5 years of experience in memory care
Background in nursing providing one-on-one care for seniors
Flexible schedule including availability to work evenings, weekends, and holidays
