• Perform thorough OASIS evaluations and develop individualized care plans
• Deliver skilled, evidence-based physical therapy in the home setting

• Educate patients and caregivers on safety, mobility, and independence

Qualifications:

• Current Pennsylvania PT license (or eligibility to obtain)
• Minimum of one year of recent home health experience
• Proficiency with OASIS documentation
• Strong organizational, time management, and communication skills
• Dedication to providing high-quality, patient-centered care
