Key Responsibilities
Complete initial assessments of patients and families to determine home care needs
Regularly re-evaluate patient nursing needs
Develop and implement a Plan of Care
Provide patient health teaching and counseling
Re-evaluate patient nursing care needs using OASIS Data Set
Coordinate all patient care and communicate with the clinical team
Document all communications and patient care activities
Participate in on-call duties and ensure availability for patient needs
Required Qualifications
High school diploma or equivalent
Graduate of an accredited school of nursing
Registered Nurse with an active unrestricted license to practice professional nursing in Texas
Current CPR certification
Valid Texas Driver’s License and auto insurance
2+ years of clinical experience in community home health or medical/surgical setting
Experience with electronic medical record application
Knowledge of nursing practice and regulations regarding home care
Ability to define problems and interpret medical instructions
Evidence of independent nursing practice
Ability to endure prolonged walking or standing
Ability to comply with professional standards
Ability to prioritize and communicate objectives
Ability to interact with multidisciplinary teams
Excellent observation, verbal and written communication skills
Basic computer skills including Microsoft Word and Outlook
Preferred Qualifications
Bachelor’s Degree in nursing
Bilingual (English/Spanish)
