As a Care Coordinator, you will connect individuals with chronic health conditions to vital services, helping them optimize their health outcomes. You'll work collaboratively to develop personalized care plans while providing crucial care management and support in both office and community settings.
Key Responsibilities
Link adults and children with chronic behavioral health and medical conditions to necessary services
Assess risk and needs, develop person-centered care plans
Provide care management services and track appointments
Educate members and coordinate health and community services
Respond to members' information and referral questions
Complete required documentation
Assist members in accessing and retaining needed services
Required Qualifications
Bachelor's degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation therapy, counseling, community mental health, child and family studies, sociology, or speech and hearing
Two years of experience in providing direct services or case management to mentally disabled or chronically ill individuals
Experience working in interdisciplinary teams
Experience providing care management or coordination in a medical or behavioral health environment
Preferred Qualifications
Fluency in a second language such as Spanish, Russian, or Creole
Specific experience with target populations for Health Home Plus or Adult Home Plus members
