HumanEdge Health is seeking an experienced and dedicated Physical Therapist for a travel outpatient assignment in Rohnert Park, CA . This opportunity is ideal for a clinician who enjoys working in a collaborative outpatient environment with a strong focus on individualized patient care.

Position Highlights:

  60-minute evaluations and treatments

• 
  

  Weekday-only schedule – no weekends or holidays

• 
  

  1:1 patient care model in a supportive team setting

• 
  

  Diverse caseload including ortho, post-op, and general outpatient

Key Responsibilities:

• 
  

  Conduct thorough patient evaluations and develop customized treatment plans

• 
  

  Implement evidence-based therapy programs to support recovery and function

• 
  

  Complete timely, accurate documentation in compliance with clinic and payer guidelines

• 
  

  Collaborate with physicians and other care providers as needed

• 
  

  Educate patients, families, and caregivers to promote continued success at home

• 
  

  Participate in ongoing clinic initiatives and professional development

Qualifications:

• 
  

  Active California Physical Therapy license – Required

• 
  

  1+ year of outpatient experience – Preferred

• 
  

  Strong manual therapy and clinical reasoning skills

• 
  

  Excellent communication and organizational abilities

• 
  

  Commitment to ethical, patient-centered care
