Key Responsibilities
Provide social casework to individuals and families receiving services
Assess family's financial situation and refer for financial assistance
Respond to referrals for casework
Participate in Agency IDG team conferences
Refer patients and families to community agencies
Interpret social resources to staff and health services
Ensure the use of the 4Ms in patient care
Required Qualifications
Master's degree (MSW) from an accredited school of Social Work
Current Social Worker License in the state of practice
Valid driver’s license and good driving record
Proficiency in clinical skills
Ability to make sound professional clinical judgment
Excellent verbal and written communication skills
Proficiency in personal computer use including email and clinical software
Preferred Qualifications
Advanced Palliative Hospice Social Worker Certification-APHSW-C
