Job Description
Join Monte Nido & Affiliates as a Social Worker and play a vital role in providing care coordination and case management across all Oregon sites. You'll support both adults and adolescents in their treatment journey, ensuring seamless access to community resources and a smooth transition between care levels.
Key Responsibilities
Care Coordination & Case Management: Partner with the clinical team to offer case management and care coordination for clients and families
Community & Family Support: Connect clients and families with community-based programs and supports
Referral Management: Send referrals to community programs and facilitate collaboration with outpatient providers
Transitions Between Care Levels: Help facilitate smooth transitions between various levels of care
Prepare Clients & Families for Transition: Provide necessary information and emotional support for transitions
Ongoing Resource Connection: Ensure clients continue to receive care after transitioning
Perform other duties as assigned
Required Qualifications
Master’s degree in Social Work or related field
Experience with CareOregon/Medicaid
Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings
Strong communication skills and ability to work collaboratively with a multidisciplinary team
Ability to build relationships with clients, families, and community providers
Understanding of available community resources, government programs, and insurance processes
Preferred Qualifications
