Job Description
Join Cooper University Health Care as a Patient Navigator and play a crucial role in bridging the gap between clinicians and patients. You will provide vital support in navigating the healthcare system, ensuring continuity of care, and enhancing patient experiences through effective communication and coordination.
Key Responsibilities
Bridge the gap between clinician and patient by providing support in navigating the healthcare system
Assist with scheduling, pre-certification process, follow-up calls, and record retrieval
Call new patients within 24 hours of their visit to check in on follow-up needs
Work with healthcare teams to support patient needs
Participate in daily multidisciplinary rounding on inpatient oncology units
Assist patients in transitioning from inpatient to outpatient care
Track patient contacts, follow-ups, barriers to care, and outcomes
Ensure delivery of patient care through customer service coordination
Required Qualifications
Experience working in a healthcare office setting or community outreach and education
Knowledge of health care, inpatient/outpatient settings, Oncology specific patient population
Strong interpersonal skills
Ability to take direction and work independently
Flexibility and cooperative working style
Preferred Qualifications
Clinical training (i.e. MA, CNA, LPN)
Bilingual Spanish/English
Competent in basic computer skills
Knowledge of using an electronic health records system
