Required Qualifications
Bachelor's Degree from an accredited Occupational Therapy Program
Master's Degree from an accredited Occupational Therapy Program

National Provider BLS - American Heart Association within 30 days of hire
Oregon Occupational Therapist License upon hire
Washington Occupational Therapist License if practicing within this state upon hire
