Job Description
Join Monte Nido & Affiliates as a Hybrid Social Worker, where you'll play a crucial role in care coordination and case management for adults and adolescents dealing with eating disorders. Help clients navigate their treatment journey while connecting them with essential community resources.
Key Responsibilities
Provide care coordination and case management for clients and families
Connect clients and families with community-based programs and resources
Manage referrals to community programs and facilitate collaboration with outpatient providers
Help facilitate transitions between various levels of care
Prepare clients and families for transitions
Ensure ongoing connection to resources post-discharge
Required Qualifications
Master’s degree in Social Work or related field
Experience with CareOregon/Medicaid
Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings
Strong communication skills
Ability to work collaboratively with a multidisciplinary team
Ability to build relationships with clients, families, and community providers
Understanding of available community resources, government programs, and insurance processes
Preferred Qualifications
LCSW preferred
