Job Description

Key Responsibilities
Perform comprehensive assessments and document findings
Ensure all diagnoses are ICD10 coded accurately
Address and document all quality elements
Conduct initial medication reviews
Travel between care sites as required
Communicate with patients and team members regarding medical concerns
Required Qualifications
Certified Nurse Practitioner through a national board or Certified Physician Assistant through a national board
Graduate of an accredited master's degree in Nursing (MSN) program
Active and unrestricted license in the state of residence
Current active DEA licensure/prescriptive authority or ability to obtain
Access to reliable transportation
Ability to lift a 30-pound bag and navigate stairs
Availability to work 24 hours per month
Preferred Qualifications
1+ years of hands-on post grad experience within Long Term Care
Understanding of Geriatrics and Chronic Illness
Understanding of Advanced Illness and end of life discussions
Proficient computer skills including electronic medical records
Ability to develop and maintain positive customer relationships
Adaptability to change
