Job Description

Join Optum as a Nurse Practitioner or Physician Assistant to provide critical care during patient transitions from rehabilitation to home, ensuring quality and compassionate healthcare. This role offers flexibility and the opportunity to make a significant impact on patient outcomes.

Key Responsibilities

  • Perform comprehensive assessments and document findings
  • Ensure all diagnoses are ICD10 coded accurately
  • Address and document all quality elements
  • Conduct initial medication reviews
  • Travel between care sites as required
  • Communicate with patients and team members regarding medical concerns

Required Qualifications

  • Certified Nurse Practitioner through a national board or Certified Physician Assistant through a national board
  • Graduate of an accredited master's degree in Nursing (MSN) program
  • Active and unrestricted license in the state of residence
  • Current active DEA licensure/prescriptive authority or ability to obtain
  • Access to reliable transportation
  • Ability to lift a 30-pound bag and navigate stairs
  • Availability to work 24 hours per month

Preferred Qualifications

  • 1+ years of hands-on post grad experience within Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including electronic medical records
  • Ability to develop and maintain positive customer relationships
  • Adaptability to change
