Job Description

Join UnitedHealthcare as a Field Care Coordinator to make a significant impact on the lives of members with medical and behavioral needs. In this role, you will engage directly with members, develop tailored care plans, and collaborate with a dedicated team to promote better health outcomes while working both remotely and in the field.

Key Responsibilities

  • Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment
  • Develop and implement person centered care plans
  • Partner and collaborate with internal care team, providers, and community resources/partners
  • Provide education and coaching to support member self-management
  • Support proactive discharge planning and manage/coordinate Care Transition
  • Advocate for members and families to ensure their needs are met

Required Qualifications

  • 2+ years of case management experience serving members with Serious Mental Illness (SMI)
  • 1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • Access to reliable transportation and the ability to travel within assigned territory

Preferred Qualifications

  • Bachelor’s degree in Psychology, Special Education, or Counseling
  • CCM certification
  • Experience working in team-based care
  • Experience in Managed Care
  • Social Work experience
  • Bilingual
