Job Description
Join UnitedHealthcare as a Field Care Coordinator to make a significant impact on the lives of members with medical and behavioral needs. In this role, you will engage directly with members, develop tailored care plans, and collaborate with a dedicated team to promote better health outcomes while working both remotely and in the field.
Key Responsibilities
Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment
Develop and implement person centered care plans
Partner and collaborate with internal care team, providers, and community resources/partners
Provide education and coaching to support member self-management
Support proactive discharge planning and manage/coordinate Care Transition
Advocate for members and families to ensure their needs are met
Required Qualifications
2+ years of case management experience serving members with Serious Mental Illness (SMI)
1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities
1+ years of experience with MS Office, including Word, Excel, and Outlook
Access to reliable transportation and the ability to travel within assigned territory
Preferred Qualifications
Bachelor’s degree in Psychology, Special Education, or Counseling
