Position Summary: The Physical Therapist (PT) provides medically prescribed physical therapy treatments for patients in the home health setting. The therapist evaluates, plans, and implements individualized care to restore function, improve mobility, relieve pain, and promote the highest level of functional independence. This position requires close collaboration with an interdisciplinary team to ensure coordinated and high-quality patient care. Key Responsibilities: Clinical Duties: Interpret and implement physician referrals and evaluate patients’ functional abilities/disabilities (e.g., range of motion, strength, coordination). Establish measurable goals, treatment plans, and timelines based on patient evaluation and medical necessity. Provide direct patient care, including individual and/or group physical therapy sessions in accordance with the plan of care. Educate patients, families, and caregivers on treatment goals, home exercise programs, and strategies to enhance independence. Participate in interdisciplinary discussions, hospital rounds, and clinics to ensure comprehensive care. Design and fabricate splints and other assistive or supportive devices as needed. Serve as a clinical resource and provide in-service training in areas of expertise within the Bronson Rehabilitation System. Participate in departmental program development and continuous quality improvement initiatives. May cross-train and provide coverage for other Rehabilitation Services departments as needed. Administrative Duties: Document evaluations, treatment plans, interventions, and progress in the Electronic Health Record (EHR) within compliance timelines. Maintain accurate, complete, and legible clinical documentation for all patients served. Communicate with leadership regarding any limitations that may impact timely documentation or care delivery. Ensure patient records reflect established goals, progress toward outcomes, and any changes to the treatment plan. Qualifications: Education: Graduate of an accredited Physical Therapy program. Licensure: Active Michigan Physical Therapist License required. Certifications: BLS (AHA) required. Experience: Minimum of one year of recent Home Health Physical Therapy experience required. Other Requirements: Valid Michigan driver’s license. Reliable vehicle available for daily business use. Proof of current auto insurance required.
