PURPOSE:
The Jewish Board's Mishkon Individualized Residential Alternatives (IRAs) and Intermediate Care Facility (ICF) provide compassionate, high quality residential and clinical services to individuals with Intellectual and/or Developmental Disabilities (IDD) in accordance with OPWDD and DOH regulations and standards. The programs ensure that the home and community-based and residential services are provided in a way that meets the religious needs of the Orthodox Jewish individuals in the programs.

The Program Coordinator contributes to the quality of resident care by overseeing and coordinating resident movement/placements and by actively ensuring that the programs are compliant with state, federal and CARF regulations and standards.

POSITION OVERVIEW:

Reporting to the Deputy Director, the Program Coordinator is a key member of the IRA/ICF management team, helping to ensure quality care, appropriate placements and overall compliance with state, federal, and CARF regulations and standards. The Program Coordinator is responsible for working with team members and staff to keep the IRAs/ICF in full compliance with regulations and guidelines and audit ready. They are the primary leaders in coordinating and overseeing resident transitions and placements, including admissions, discharges and returns from hospital, rehab or nursing home stays, as well as additional communication to help engage and coordinate care with family members. They actively partner and collaborate with colleagues and staff, as well as with external stakeholders, including families, advocates with OPWDD and DOH.

KEY ESSENTIAL FUNCTIONS:
Responsible lead, in partnership with IRA/ICF management team, for keeping the IRAs/ICF audit ready and in compliance with state, federal and CARF regulations, guidelines and standards of care
• Knowledgeable of state, federal and CARF regulations and standards of care, working with Compliance and other sources to build understanding and to keep up-to-date with revisions.
• Develops, maintains, reviews and reports out on weekly data and trackers.
• Conduct chart and documentation reviews.
• Participates in the development of Plans of Corrective Action (POCA) in response to external audits by OPWDD, DOH or other regulatory bodies, and executes and monitors implementation of the POCA.
• Helps develop and implement internal corrective actions in response to internal findings, observations and audits.
• Assists in preparation for and facilitation of external audits and CARF accreditation.
• Tracks and ensures completion of required training for IRA/ICF staff, in collaboration with IRA/ICF management team members.
• Track incidents, ensuring timely follow up on incident recommendations and closure.
Primary leader coordinating and overseeing resident care, transitions and placements, including admissions, discharges and returns from hospital, rehab or nursing home stays
• Co-facilitate Admission/Discharge Committee.
• Tracks admission, discharges, out-of-resident stays in hospital, rehab or nursing home, participating in related meetings.
• Coordinates plans for appropriate placement based on individual need and setting internally with medical, nursing, clinical and residential and externally with hospitals/rehab/nursing homes, families, advocates and OPWDD.
• Facilitates meetings and communication, as required.
• Documents contacts and plans.
• Escalates challenging resident care and placement situations up for division and agency support.
• Facilitates and tracks all required steps for admission and discharges to the IRAs/ICF.
Key member of IRA/ICF management team
• Coordinates care across disciplines and programs in a respectful, effective and collaborative manner.
• Assists with and responds to emergency situations.
• Provides coverage for vacant positions or for colleagues PTO, on a temporary and as needed basis.
• Implements and supports system changes.
• Utilizes data and proven performance improvement tools, such as Lean Six Sigma, to identify and solve problems.
• Participates, with the IRA/ICF management team, in meetings, communication, planning and operations.
• Travels to IRAs/ICF, as needed.
• Other duties as assigned.

EDUCATIONAL / TRAINING REQUIRED:
• Master's degree in a relevant field required, OR advanced certification in healthcare management, administration or quality improvement.

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE:
• A minimum of 5 years of experience working with the intellectually disabled population.
• A minimum of 2 years in a supervisory, compliance or quality role.
• Knowledge of OPWDD and DOH regulations.
• Hebrew, Yiddish or Russian-speaking preferred.

COMPUTER SKILLS REQUIRED:
• Strong computer skills are required. The ability to use Microsoft applications including Word, Excel, and PowerPoint are critical. Familiarity with electronic health records.

VISUAL AND MANUAL DEXIERITY:
• The visual and manual dexterity needed for this position include the ability to work on the computer typing reports and reviewing charts. There is also the need to speak frequently on the telephone, including a cellular phone and participate in video conference meetings.

WORK ENVIRONMENT / PHYSICAL EFFORT:
• This position requires the ability to visit multiple homes daily. It involves walking within residential programs, including navigating stairs and interacting with residents.
• Requires movement to respond to crises or emergencies, which includes walking, sitting, standing, and bending as necessary, and sometimes quickly, to provide care and intervene effectively in various settings.
• Regularly required to sit for part of the day.
• The lighting and noise levels are consistent with those typically found in a healthcare or long-term care environment.
• Must be able to lift up to 20 lbs.
