MedAdventures is partnering with a home health agency serving Cuyahoga and Medina Counties to find a PT who's ready to transition from traditional settings (like outpatient, SNF, or acute care) into the rewarding world of home-based care .

You’ll receive support and training on OASIS assessments and documentation, so even if you haven’t worked in home health before, we’ll help you make the transition with confidence.

What You’ll Do:

• Provide in-home physical therapy in Cuyahoga & Medina Counties
• Complete evaluations and personalized treatment plans
• Educate patients and caregivers on exercises and fall prevention
• Document visits and assessments using OASIS
• Collaborate with a multidisciplinary care team

What You’ll Need:

• Active Ohio PT license
• Reliable transportation
• Strong communication & time management skills
• A patient-centered approach and openness to learning a new care model
