Key Responsibilities
Direct care and case management of assigned clients
Assist regional director and personnel to identify and improve services
Plan, implement, and evaluate care provided
Manage client care conferences
Serve as a local on-site clinical resource
Collaborate with staffing coordinators
Ensure adherence to Agency policies and guidelines
Participate in interviewing and training of clinical personnel
Review clinical documentation for accuracy
Participate in performance improvement initiatives
Required Qualifications
Graduate of a state approved school of professional registered nursing
Current, unrestricted RN license in the state(s) of practice
Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
Current CPR certification
Preferred Qualifications
BSN preferred
One (1) year of supervisory and/or case management experience preferred
