Travel RN – Case Management / Utilization Review
Location: The Bronx, NY
Start Date: ASAP
Assignment Length: 13 weeks
Shift: Days (5 x 8.5 hr shifts with 1-hour lunch)
Weekly Hours: 37.5
Unit Type: Case Management (Utilization Review) in an acute care, Level II trauma setting
Electronic Medical Record: Allscripts
Role Overview:
As a Case Management RN, you’ll independently manage discharge planning and referrals for 17–25 patients on med‑surg or telemetry floors. You’ll work closely with social workers and the interdisciplinary care team, rounding with physicians and ensuring patients transition safely—whether to home, community resources, or skilled nursing facilities. Your role also involves coordinating housing referrals, wound vac services, dialysis scheduling, and social services for vulnerable populations.
Requirements:
Active NYS RN license required at time of submission
BSN degree required
Minimum 1 year of inpatient case management experience (critical care background preferred)
Certifications at submission:
BLS (AHA only)
PRI (Patient Review Instrument) Certification
SCREEN Training Certification
Experience with utilization review tools preferred (e.g. InterQual, MCG, CarePort)
Work Environment & Expectations:
Caseload of 17–25 patients, split with social workers (pilot floors have fewer patients per nurse)
Occasionally cover weekends and holidays with manager approval
