Join our team as a Senior Case Manager RN, where you can create your own schedule and make a difference in the lives of former Nuclear Weapons Workers. You will lead a team of RN Case Managers providing in-home care, while ensuring adherence to agency policies and improving client services.
Key Responsibilities
Direct care and case management of assigned clients
Plan, implement, and evaluate care provided
Coordinate and manage client care conferences
Ensure adherence to Agency policies
Assist in interviewing, selection, and ongoing evaluation of clinical personnel
Participate in performance improvement initiatives
Required Qualifications
Graduate of a state approved school of professional registered nursing
Current, unrestricted RN license in the state(s) of practice
Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
Current CPR certification
Preferred Qualifications
BSN preferred
One (1) year of supervisory and/or case management experience preferred
