This Community Health Worker Training Program equips participants with essential skills to bridge healthcare and social services for underserved communities. After completing 550 hours of training, you'll be prepared to apply for CHW licensure in Oregon and make a significant impact in your community.
Key Responsibilities
Establish trusting relationships with patients and families
Coach patients in effective management of chronic health conditions
Assist patients in understanding care plans
Visit patients to assess living conditions
Document activities and service plans
Advocate for patients with community service agencies
Help patients access health-related services
Facilitate communication between providers
Motivate patients to engage in their health
Work with diverse backgrounds to reduce barriers
Required Qualifications
Must be 18 years or older
Must live within 30-45 minutes from Gresham, OR
Must have a high school diploma or GED
Must be a U.S. Citizen or Permanent Resident
Must be able to pass a national criminal background check
Preferred Qualifications
Familiarity with Microsoft Office Suite (Word, Excel, PowerPoint)
