Key Responsibilities
Develop and implement care plan interventions
Communicate health-related information to stakeholders
Advocate for patients and families
Assess and implement individualized care strategies
Identify problems/barriers to care
Initiate referrals for social service programs
Provide resource support for local services
Manage person-centered service/support plans
Conduct home visits and in-person visits
Maintain compliance with state and federal guidelines
Required Qualifications
Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan
Master’s degree with current, unrestricted independent licensure as a Social Worker (LMSW, LCSW, LLMSW)
Bachelor’s degree with current, unrestricted independent licensure as a Social Worker (LLBSW, LBSW)
2+ years of experience in community health settings
1+ years of experience with local behavioral health providers
1+ years of experience working with long-term care needs
1+ years of experience with electronic documentation systems and MS Office
Access to reliable transportation for member visits
Access to a designated workspace for remote work
Ability to travel to Southfield, MI for meetings
Must reside within the state of Michigan
Preferred Qualifications
RN or LMSW; LCSW, LLMSW
1+ years of medical case management experience
Training or certifications in Motivational Interviewing and Trauma-Informed Care
Experience with individuals with co-occurring disorders
Experience with MI Health Link (MMP)
Experience in Managed Care
Knowledge of NCQA documentation standards
