Key Responsibilities

  • Develop and implement care plan interventions
  • Communicate health-related information to stakeholders
  • Advocate for patients and families
  • Assess and implement individualized care strategies
  • Identify problems/barriers to care
  • Initiate referrals for social service programs
  • Provide resource support for local services
  • Manage person-centered service/support plans
  • Conduct home visits and in-person visits
  • Maintain compliance with state and federal guidelines

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan
  • Master’s degree with current, unrestricted independent licensure as a Social Worker (LMSW, LCSW, LLMSW)
  • Bachelor’s degree with current, unrestricted independent licensure as a Social Worker (LLBSW, LBSW)
  • 2+ years of experience in community health settings
  • 1+ years of experience with local behavioral health providers
  • 1+ years of experience working with long-term care needs
  • 1+ years of experience with electronic documentation systems and MS Office
  • Access to reliable transportation for member visits
  • Access to a designated workspace for remote work
  • Ability to travel to Southfield, MI for meetings
  • Must reside within the state of Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Training or certifications in Motivational Interviewing and Trauma-Informed Care
  • Experience with individuals with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
