Travel Pay Rate : $2,920 weekly ($30.5/hr W2 + $1700/week stipend)

Job Description:  

We're urgently seeking an experienced Registered Nurse Case Manager for a travel contract in Santa Rosa, CA. We're looking for nurses who have a minimum of 2 years of recent acute hospital Case Management experience, specializing in Care Coordination and Discharge Planning within a fast-paced environment. Proficiency with the Epic charting system is required, and candidates must be prepared for a Day shift (5x8-hour) schedule with weekend coverage (2 weekends every 6 weeks). This is a travel-only position; locals will not be accepted.

Expected ratios 1:20

Responsibilities:

• Plan Discharges (get patients home or to next facility).
• Coordinate patient care and necessary services.
• Arrange things like wheelchairs (DME) and ambulance rides.
• Check if patient stay is medically necessary using guidelines (InterQual).
• Run family meetings about treatment plans.
• Handle insurance approvals and benefits checks.
• Ensure all patient documentation follows legal rules (HIPAA/CMS).
• Use the Epic computer system for charting.
• Float to other units if needed by the hospital.
• Work every other weekend .

Requirements:

• 2 years Experience as an RN Case Manager
• BLS, PALS (AHA only)
• RN State License of CA
• EPIC experience - required;
