Key Responsibilities
Provide direct clinical services to patients in a hybrid care model
Screen and assess patients for mental health and substance abuse disorders
Track treatment response and monitor clinical symptoms
Support medication management and treatment adherence
Provide brief behavioral interventions using evidence-based techniques
Participate in caseload consultation with psychiatric consultants
Develop relapse prevention self-management plans for patients
Required Qualifications
Licensed Clinical Social Worker (LCSW)
Licensed Master Social Worker (LMSW)
Licensed Advanced Practice Social Worker (LAPSW)
Licensed Mental Health Counselor (LMHC)
Licensed Professional Counselor - MHSP (LPC-MHSP)
Licensed Marriage and Family Therapist (LMFT)
3+ years of experience treating patients with behavioral health conditions
1+ years of experience working in primary care or medical behavioral integrated setting
Experience with assessment and evidence-based psychosocial treatments
Preferred Qualifications
Experience in the Collaborative Care Model (CoCM)
Detail-oriented and organized
Effective professional relationship maintenance
Ability to work with patients in person and via telehealth
Solid grasp of technology solutions and tools
