Key Responsibilities

• Responds to social needs and issues in the Emergency Department and inpatient services
• Completes biopsychosocial assessments
• Provides trauma-informed, strengths-based interventions
• Screens and assesses patients for social determinants of health
• Provides case management and advocacy
• Collaborates with multi-disciplinary teams
• Develops safety plans with patients
• Provides crisis intervention and problem-solving support
• Communicates effectively with healthcare professionals
• Maintains therapeutic relationships with patients and families
