Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. You'll work closely with clients, providing essential support and resources to help them lead independent lives.
Key Responsibilities
Function as a case planner and provide appropriate interventions for client care
Develop action plans utilizing existing and new resources
Conduct routine community contacts and evaluations of clients
Provide individual/group counseling based on client needs
Perform home visits to assess living conditions and needs
Facilitate referrals to treatment/service providers
Manage client cases and document necessary paperwork
Required Qualifications
BA Degree in social services or related field
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
