Position Summary:

VitalHire is seeking a skilled and compassionate PTA - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Nashua, NH. Key Responsibilities:

• Provide physical therapy services under supervision of licensed Physical Therapist
• Implement treatment plans and therapeutic interventions for patients in skilled nursing facility setting
• Assist patients with therapeutic exercises and mobility training
• Document patient progress and maintain accurate treatment records
• Communicate effectively with patients, families, and healthcare team members
• Ensure patient safety during all therapeutic activities
• Maintain professional standards and adhere to facility policies and procedures
• Participate in interdisciplinary team meetings and care planning
• Follow infection control protocols and safety guidelines
• Complete required documentation in a timely and accurate manner
• Assist with patient transfers and positioning as needed
• Maintain current state licensure and required certifications
• Work scheduled shifts from 08:00 to 16:30

Job Requirements:

• Active, unencumbered license in NH
• Minimum of 1-7 years of relevant experience, depending on specialty
• Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
• Experience with EMR systems (Epic, Cerner, as applicable)

Other Required Documentation:

• Current Resume listing detailed work history (MM/YYYY format)
• Two current clinical references
• State-required professional license
• Physicial exam (completed within the past 12 months)
• TB skin test or Quantiferon Gold (within the past 12 months)
• Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
• Current Tdap and Flu vaccinations
• Must pass background check and drug screening
