Job Description
The MDS Coordinator (RN) oversees the RAI process, conducts assessments, and coordinates care plans for residents. This leadership role involves supervising nursing staff and requires expertise in the MDS process.
Key Responsibilities
Completes the MDS, CAA’s and care plans within regulated time frames.
Coordinates scheduling the RAI process with the interdisciplinary team.
Assesses resident through physical assessment, interview and chart review.
Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
Reviews information from hospital, consults and outside agencies for assessment and care planning.
Conducts PPS meetings maintaining MDS assessments per Medicare schedule.
Remains current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
Case management.
Required Qualifications
Registered Nurse (RN) in the state
Preferred Qualifications
AANC certification
RAC-CT certification
Knowledge of the Resident Assessment Instrument (RAI) process
Experience as an MDS Nurse
