Key Responsibilities
Provide supportive services/counseling on healthcare and home care programs
Serve as a member of the interdisciplinary team
Provide crisis intervention and assist families
Coordinate healthcare programs among patients and healthcare teams
Respond to suspected abuse, neglect, or violence
Develop appropriate plan of care for patients and families
Participate in care conferences and transitional rounding
Required Qualifications
Bachelor’s degree in Social Work from an accredited curriculum
Licensed Social Worker (LSW) in Minnesota
Graduate from a nationally accredited nursing program
Bachelor’s Degree in nursing or completion within 18 months of hire
Currently holds an unencumbered RN license
