Key Responsibilities
Evaluates and treats patients in accordance with care standards
Monitors patient care using available data and chart reviews
Coordinates patient services including specialty referrals and home health care
Participates and provides medical expertise in daily care team huddles
Completes medical record documentation in a timely manner
Participates in quality of care discussions with Regional Medical Director
Assures compliance with licensing and accrediting bodies
Focuses 100% of time on direct patient care
Required Qualifications
Current and unrestricted medical license or willing to obtain
Graduate of accredited MD or DO program
Excellent verbal and written communication skills
High level of skill with interpersonal relationships
Fully engaged in the concept of Integrated team-based care
Willingness and ability to learn/adapt to value-based care
Superior patient/customer service
Basic computer skills, including email and EMR
Patient-facing role requiring TB screening
Preferred Qualifications
Board Certification or Eligible to become certified in Family Medicine, Internal Medicine or Geriatric Medicine
Active and unrestricted DEA license
Medicare Provider Number
2-5 years of directly applicable experience
Experience managing Medicare Advantage panel
Knowledge of Medicare guidelines and coverage
Bilingual is a plus
Knowledge of HEDIS quality indicators
