Job Description
As a Case Manager II at the Seattle Indian Health Board, you will provide holistic care by assisting relatives in navigating community resources, developing care plans, and promoting safety. This role emphasizes collaboration with interdisciplinary teams to ensure the best outcomes for clients, all while upholding Indigenous values and practices.
Key Responsibilities
Perform client intake and assess safety and needs
Assist relatives in developing care and self-sufficiency plans
Help obtain community resources and educate on safety practices
Conduct education and support groups for survivors
Participate in community outreach events
Collaborate with interdisciplinary teams for referrals
Develop and document case management plans and maintain records
Attend mandatory meetings and complete required reports
Required Qualifications
Master’s Degree in social work or related field
2+ years’ experience providing direct case management
Current WA State Driver’s License
Preferred Qualifications
Knowledge of health, social, and DV/GBV issues and services faced among AI/AN community in King County
