Key Responsibilities

  • Conduct comprehensive bio-psychosocial assessments and establish care plans
  • Collaborate with interdisciplinary teams to assess level of care and develop care plans
  • Utilize therapeutic methods in accordance with care plans
  • Monitor progress of interventions and revise plans as needed
  • Provide education to patients/families on health management
  • Deliver direct patient care including case management and counseling
  • Refer patients to appropriate care levels and resources
  • Maintain timely patient-centered documentation
  • Perform clinical diagnosis and establish care plans
  • Provide in-home treatment focusing on parent/infant attachment
  • Participate in administrative activities including clinical supervision
  • Provide supervision to second year social work students
  • Support development of hospital programming
  • Contribute to improved patient experience
  • Ensure cost-effective utilization of resources

Required Qualifications

  • Master's Degree in Social Work, Counseling, Marriage and Family Therapy or Related Human Service Field
  • Previous experience in a healthcare setting
  • LCSW, LPC, LMFT
  • Valid Driver's License
  • Computer proficient
  • Ability to assess problems, determine solutions and make decisions in a timely manner
  • Excellent communication skills
  • Demonstrates competencies to meet age specific patient needs
  • Focused on personal, patient/client and unit/staff safety
  • Assumes responsibility for self-improvement
  • Cognizant of health-related social needs and cultural competence
  • Develops and maintains cultural competence
