Key Responsibilities
Conduct Adult/Geriatric assessments including medical history and treatment
Collaborate with healthcare providers and case managers
Provide follow-up and patient education
Perform home visits and skilled facility visits
Required Qualifications
Master’s Degree or commensurate experience
Current RN and NP licensure in state of practice
Board certified by AANP, ANCC, or AACCN
Current BLS, ACLS, or CPR certification
1 year experience as a Nurse Practitioner preferred
