Join Primary Vascular Care as a Nurse Practitioner or Physician Assistant, delivering critical on-site clinical assessments in a long-term care setting. This role requires independent functioning to optimize patient treatment plans and ensure compliance with regulations, all while working 30-40 hours a week.
Key Responsibilities
Perform on-site clinical assessments to determine patient treatment plans
Function independently to perform history and physical for patients
Order and review diagnostic and therapeutic tests
Assist with care provision in accordance with regulations
Monitor the effectiveness of interventions
Maintain relationships with facility contacts
Educate family members and clients on services and programs
Assist with physician relations development
Complete reports in a timely manner
Required Qualifications
Bachelor's Degree
Degree from an accredited Nurse Practitioner or Physician Assistant Program
Current NP License
BLS - Basic Life Support certification
Current Infection Control Certification
Board Certified/Board Eligible
Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella
Preferred Qualifications
ACLS - Advanced Cardiac Life Support certification
