Job Description

Join Primary Vascular Care as a Nurse Practitioner or Physician Assistant, delivering critical on-site clinical assessments in a long-term care setting. This role requires independent functioning to optimize patient treatment plans and ensure compliance with regulations, all while working 30-40 hours a week.

Key Responsibilities

  • Perform on-site clinical assessments to determine patient treatment plans
  • Function independently to perform history and physical for patients
  • Order and review diagnostic and therapeutic tests
  • Assist with care provision in accordance with regulations
  • Monitor the effectiveness of interventions
  • Maintain relationships with facility contacts
  • Educate family members and clients on services and programs
  • Assist with physician relations development
  • Complete reports in a timely manner

Required Qualifications

  • Bachelor's Degree
  • Degree from an accredited Nurse Practitioner or Physician Assistant Program
  • Current NP License
  • BLS - Basic Life Support certification
  • Current Infection Control Certification
  • Board Certified/Board Eligible
  • Physical, Vaccines and Titers: PPD/TB, Hep B, MMR, Varicella

Preferred Qualifications

  • ACLS - Advanced Cardiac Life Support certification
  • COVID and Flu Vaccine
  • 1 year of NP experience
