Job Description

Key Responsibilities
Provides counseling to individuals and families throughout the cancer experience
Participates in identifying psychosocial needs of cancer patients/families
Facilitates a cancer support group and other psychosocial functions
Identifies out-patients with complex discharge planning needs
Acts as a consultant for in-patients with complex discharge planning needs
Documents activities in the medical record
Assesses need for grant-funded transportation services
Serves as liaison to external providers for post treatment services
Facilitates transfer of information among healthcare professionals
Identifies patients with special age-specific needs and makes referrals
Identifies patients with financial needs and facilitates access to services
Required Qualifications
Licensed to practice as a Social Worker in the District of Columbia
Masters degree in Social Work from an accredited school
Minimum of 2-5 years social work experience in a health care setting
Current CPR Certification
