Key Responsibilities

  • Provides counseling to individuals and families throughout the cancer experience
  • Participates in identifying psychosocial needs of cancer patients/families
  • Facilitates a cancer support group and other psychosocial functions
  • Identifies out-patients with complex discharge planning needs
  • Acts as a consultant for in-patients with complex discharge planning needs
  • Documents activities in the medical record
  • Assesses need for grant-funded transportation services
  • Serves as liaison to external providers for post treatment services
  • Facilitates transfer of information among healthcare professionals
  • Identifies patients with special age-specific needs and makes referrals
  • Identifies patients with financial needs and facilitates access to services

Required Qualifications

  • Licensed to practice as a Social Worker in the District of Columbia
  • Masters degree in Social Work from an accredited school
  • Minimum of 2-5 years social work experience in a health care setting
  • Current CPR Certification
