The Physical Therapist is responsible for evaluating residents, developing personalized treatment plans, and delivering physical therapy services in a skilled nursing facility. The PT works to improve residents’ strength, mobility, balance, and overall physical function, with the goal of enhancing independence and quality of life. All services must comply with federal, state (Colorado), and facility regulations.

Perform initial and ongoing assessments of resident physical function and mobility.
Develop, implement, and revise individualized treatment plans based on assessment results.
Administer physical therapy treatments and interventions in line with best practices.
Educate residents, families, and staff on proper techniques and home exercise programs.
Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely.
Collaborate with the interdisciplinary care team, attending care conferences and assisting with discharge planning.
Supervise and delegate duties to Physical Therapist Assistants (PTAs) as required by Colorado licensure laws.
Ensure compliance with Medicare, Medicaid, and facility documentation and billing standards.
Maintain equipment and work areas to ensure safety and cleanliness.
Doctorate or Master’s degree in Physical Therapy from an accredited institution.
Current Colorado Physical Therapist License in good standing.
Valid CPR certification.
Previous experience in skilled nursing, long-term care, or geriatric rehab preferred.
Strong interpersonal, documentation, and organizational skills.
Ability to work effectively in a team-oriented, resident-centered environment.
Must be able to lift up to 50 pounds.
Frequent standing, walking, reaching, and assisting with resident transfers or ambulation.
Ability to perform tasks that require physical stamina and mobility in a clinical setting.
Competitive salary $40/hr - $50/hr DOE
