Job Summary
A skilled and dedicated Physical Therapy Assistant is needed to provide high-quality home health therapy services across designated territories. A minimum of one (1) year of professional PTA experience is required; prior home health experience is highly preferred. This role involves delivering patient-centered care in a home-based setting, working independently, and managing a flexible caseload.
Territory Coverage
Primary area includes Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City , plus either :
- Area #2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont
OR - Area #3: San Ramon, Dublin, Pleasanton, Sunol, Livermore
Scheduling Options
Two assignment structures are available: Guaranteed Hours or Non-Guaranteed Hours.
Guaranteed Hours
- 36 hours per week guaranteed
- Minimum productivity: 30 points/week
- Must be willing to travel outside assigned area or work weekends if needed
- Guarantee does not apply if:
- Patients are refused
- It is the first or last week of the assignment
- A holiday occurs during the week
- Approved time off is taken
Non-Guaranteed Hours
- No guaranteed hours
- Weekend coverage may be required
- Occasional travel outside primary territory may be necessary
- May refuse patients
- Must be willing to see COVID+ and PUI patients
- Paid per-visit , with visit types assigned point values
Visit Point System
- Routine Visit = 1 point = 1.33 hours
Hours are approved based on points earned per day.
Point system accounts for: - Drive time
- Documentation
- Patient & physician calls
- Visit time
Role Responsibilities
- Provide high-quality physical therapy treatments under the direction of a licensed PT
- Complete timely documentation and next-day reporting for start-of-care visits
- Communicate effectively with supervisors, intake teams, and office staff
- Supervise Physical Therapy Assistants when required
- Ensure all clinical notes are completed within 48 hours
- Maintain responsiveness during business hours
- Travel throughout assigned territories to perform home health visits
Required Credentials
- Active California PTA License
- BLS Certification
- Valid driver’s license
- Auto insurance & vehicle registration
- Current TB test
- Immunizations: MMR, Varicella, HepB, Tdap (10 years), Flu (annual)
- Drug screen & physical
Compliance Requirements
- All requested time off must be disclosed at submission
- No RTO will be approved after acceptance
- Traveler contract must match the client confirmation exactly
- Non-compliance may result in vendor removal
