Key Responsibilities
Carry a complex caseload and practice person-centered service planning
Evaluate and assess residents' independent living skills
Advocate for residents and interpret agency policies
Complete timely documentation of resident interactions
Participate in intake, assessments, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
