Join Molina Healthcare as a Care Manager, where you will work collaboratively with members and healthcare teams to coordinate integrated care for individuals with high needs. This role involves clinical assessments, case management planning, and facilitating interdisciplinary collaboration to ensure quality care and improved health outcomes.
Key Responsibilities
Complete clinical assessments of members and determine eligibility for case management
Develop and implement case management plans in collaboration with members and healthcare professionals
Conduct telephonic or face-to-face visits as required
Monitor care plans and document interventions and goal achievements
Maintain a member case load for ongoing outreach and management
Promote integration of services for enhanced continuity of care
Facilitate interdisciplinary care team meetings
Use motivational interviewing to educate and support members
Assess barriers to care and provide appropriate coordination
Required Qualifications
Bachelor's or Master's Degree in social science, psychology, gerontology, public health, social work or related field
1-3 years in case management, disease management, managed care or medical or behavioral health settings
Active, unrestricted license if required for the job
Valid driver’s license with good driving record
Preferred Qualifications
3-5 years in case management, disease management, managed care or medical or behavioral health settings
Licensed Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Case Manager (CCM), Certified in Health Education and Promotion (CHEP), Licensed Professional Counselor (LPC/LPCC), Respiratory Therapist, or Licensed Marriage and Family Therapist (LMFT)
