Job Description
Join The Jewish Board as a Case Associate, where you'll make a significant impact by supporting clients in leading safe and independent lives. You'll develop action plans, conduct home visits, and connect clients with essential resources while working within a dedicated team focused on community service.
Key Responsibilities
Function as a case planner and supply appropriate interventions for client care
Develop action plans within the client's community
Conduct routine community contacts and evaluations
Perform home visits to assess living conditions
Provide case management and guidance in independent living skills
Identify and facilitate referrals to treatment or service providers
Run individual/group counseling sessions
Conduct community outreach
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
