Key Responsibilities
Evaluates and treats center patients according to standards of care
Monitors care using available data and chart reviews
Assists in coordinating patient services
Participates in daily huddles with care team
Completes medical record documentation timely
Follows policy and protocol defined by Clinical Leadership
Participates in potential growth opportunities for services
Meets with RMD regarding quality of care and outcome data
Participates in local primary care on-call program
Ensures compliance with licensing and accrediting bodies
Focuses on direct patient care and related administrative tasks
Required Qualifications
Current and unrestricted medical license
Graduate of accredited MD or DO program
Excellent verbal and written communication skills
High level of skill with interpersonal relationships
Engaged in integrated team-based care model
Willingness to learn/adapt to value-based care
Superior patient/customer service skills
Basic computer skills including email and EMR
Preferred Qualifications
Board Certification in Family Medicine, Internal Medicine or Geriatric Medicine
Active and unrestricted DEA license
Medicare Provider Number
2-5 years of directly applicable experience
Experience managing Medicare Advantage panel
Knowledge of Medicare guidelines and coverage
Bilingual
Knowledge of HEDIS quality indicators
