Job Description:
We are seeking a highly motivated Physical Therapist (PT) – Home Health to join our team in Pittsfield, MA. The PT will provide skilled physical therapy services to patients in their homes, focusing on restoring mobility, enhancing independence, and ensuring quality outcomes. This role requires the ability to work independently while delivering compassionate, patient-centered care.

Submission Requirements:

• 2+ years of acute nursing care (hospital-based) and home care experience: Required
• Certified home health experience including OASIS proficiency: Required, Non-Negotiable
• Valid Driver’s License: Required
• Must be able to work independently with minimal supervision
• Acute care experience: Preferred
• Two specific interview dates/times must be included in profile (no “anytime” accepted)
• Traveler permanent address must be included (facility will not consider candidates within 50 miles of the facility)

• 1 supervisory signed reference (within past 2 years)
