Job Description

As a Field Foster Care Coordinator at UnitedHealthcare, you will leverage trauma-informed approaches to support children and youth in the foster care system. This role requires coordination of care strategies, advocacy, and collaboration with multiple stakeholders to ensure access to necessary services. Your work will have a lasting impact on the lives of vulnerable families in your community.

Key Responsibilities

  • Assess, plan and implement individualized care strategies for children and youth
  • Identify, initiate, coordinate, and monitor referrals for psychosocial, behavioral, and medical services
  • Manage the care plan throughout the continuum of care
  • Communicate with stakeholders to ensure quality coordinated care
  • Advocate for children, youth, and families within the healthcare system
  • Provide trauma-informed education to members and caregivers
  • Coordinate with child welfare and juvenile justice systems
  • Coordinate additional services including educational advocacy and early childhood intervention

Required Qualifications

  • Master’s Degree in Social Work, Nursing, Psychology or related human services area
  • Unrestricted, independent licensure as a Registered Nurse
  • Bachelor’s Degree in Social Work, Rehabilitation, Nursing, Psychology, Special Education, Gerontology, or related human services area
  • 2+ years of experience working within the community health setting in a healthcare role
  • 1+ years of experience working with multi-system children
  • 1+ years of experience working with MS Word, Excel and Outlook
  • Ability to travel in assigned regions to visit Medicaid members
  • Access to reliable transportation & valid US driver's license

Preferred Qualifications

  • Licensed Social Worker or clinical degree
  • Experience with electronic charting
  • Experience with arranging community resources
  • Background in managing populations with complex medical or behavioral needs
