You will support clients and their families in navigating essential community resources while ensuring seamless transitions in care.

Required Qualifications
Master’s degree in Social Work or related field
Experience with CareOregon/Medicaid
Proven experience in case management or care coordination, preferably in behavioral health or residential treatment settings
Strong communication skills
Ability to work collaboratively with a multidisciplinary team
Ability to build relationships with clients, families, and community providers
Strong understanding of available community resources, government programs, and insurance processes
Preferred Qualifications
LCSW preferred
