Join Optum as an Associate Patient Care Coordinator to make a significant impact on patient health outcomes through effective communication and scheduling. You will play a vital role in connecting patients with essential health services, ensuring they receive timely and quality care.
Key Responsibilities
Patient telephonic outreach to schedule patients for PSU health events
Collect and verify demographic information and insurance eligibility
Timely notification and reminder calls for upcoming appointments
Follow-up with patients regarding cancelled or rescheduled appointments
Enter dispositions for calls made to patients in the system
Monitor call numbers to ensure no more than six calls within 60 days
Use telephonic software for outreach and assist inbound calls
Follow approved scripts for outreach calls
Support individual PSU provider requests to schedule follow-up appointments
Direct and follow-up on patient messages and concerns
Meet established daily productivity goals
Establish professional working relationships with patients and staff
Perform all other related duties as assigned
Required Qualifications
High School Diploma/GED (or higher)
1+ years of experience with general office procedures
Advanced level of proficiency with communication and customer service skills both in person and via phone
Basic level of proficiency with Microsoft applications
Access to reliable transportation & valid US driver’s license
Preferred Qualifications
1+ years of related work experience including data entry
ICD 10 and CPT coding experience
Prior experience with EMR computer applications
Working knowledge of medical terminology
Bilingual in English & Spanish
