Registered Nurse (RN) – Case Management Job Summary

The Registered Nurse Case Manager provides comprehensive support and expertise in case management, utilization review, and discharge planning . This role ensures quality patient care through ongoing assessment, planning, implementation, and evaluation of individual patient needs. The RN Case Manager promotes continuity of care, cost-effectiveness, and optimal patient outcomes while maintaining compliance with all applicable regulations and facility policies.

Key Responsibilities

• Conduct comprehensive assessments of patient needs for appropriate care coordination.
• Develop and implement individualized discharge plans to ensure safe transitions of care.
• Perform utilization review to monitor the appropriateness of admissions, continued stays, and services provided.
• Collaborate with physicians, interdisciplinary teams, patients, and families to achieve care goals.
• Document and maintain accurate records according to facility and regulatory standards.
• Participate in performance improvement and compliance activities.

Qualifications

Education:

• Graduate of an accredited school of nursing.
• BSN preferred.

Experience:

• Minimum of 4 years of experience as a Registered Nurse.
• Minimum of 2 years of acute hospital experience required.
• EPIC experience required.
• Previous experience in case management, utilization review, or discharge planning preferred.
• No first-time travelers.

Licensure/Certification:

• Active Registered Nurse (RN) license in the state of North Carolina (or compact state).
• BLS certification required.
• ACM or CCM certification preferred.
