Key Responsibilities
Conduct psychosocial evaluations of patients and families
Provide ongoing counseling during the end-of-life journey
Offer emotional support in accordance with the plan of care
Collaborate with the healthcare team to ensure comprehensive patient care
Required Qualifications
MS degree in social work from an accredited school
Current state license as a social worker
2+ years of clinical work experience, preferably in healthcare or hospice
Reliable transportation
Ability to sit, stand, bend, move intermittently and lift at least 25lbs
