Job Description

Join Optum as a Field Clinical Practice Performance Specialist, where you'll improve health outcomes by coordinating care and supporting members' wellness. This role, based in Omaha, involves reviewing charts, scheduling appointments, and collaborating with healthcare teams to ensure members receive the best care possible.

Key Responsibilities

  • Provide care coordination through physician practices
  • Conduct telephonic member needs assessments
  • Interact with members via telephone and schedule appointments
  • Review member charts prior to physician appointments
  • Partner with practice staff to manage member appointments
  • Create and maintain professional relationships
  • Facilitate member referrals to special programs
  • Assist members in accessing community and Medicare resources
  • Collaborate with interdisciplinary teams to coordinate care
  • Provide education on health care needs and available services
  • Work to facilitate member compliance with health plans
  • Identify barriers for compliance and communicate with members
  • Document member conversations and activities
  • Maintain focus on timely, high-quality customer service
  • Ensure confidentiality of sensitive information

Required Qualifications

  • High School Diploma/GED (or higher)
  • 2+ years of experience in a hospital, acute care, or healthcare
  • 2+ years of customer service/call center experience
  • Intermediate computer/typing proficiency
  • Ability to travel to provider offices within a 2-hour radius
  • Access to reliable transportation and a valid US driver’s license

Preferred Qualifications

  • Associates degree or higher
  • Problem-solving skills
  • Experience with HEDIS and EMR
  • Experience navigating and analyzing reports in Excel
  • Medicaid, Medicare, Managed Care experience
  • Prior Physician office experience
  • Experience with vulnerable populations
  • Call Center experience
  • Excellent verbal and written communication skills
  • Demonstrated ability to empathize with customers
