Key Responsibilities
Evaluate and treat center patients according to standards of care
Monitor care quality using data and chart reviews
Coordinate patient services including referrals and home health care
Participate in daily huddles with the care team
Assist in setting a cooperative tone in practice
Complete medical record documentation timely
Participate in quality of care discussions with leadership
Engage in growth opportunities for services within the center
Participate in local primary care 'on-call' program
Ensure compliance with licensing and accrediting bodies
Focus 100% of time on direct patient care
Required Qualifications
Current and unrestricted medical license or willing to obtain
Graduate of accredited MD or DO program
Excellent verbal and written communication skills
High level of skill with interpersonal relationships
Engagement in integrated team-based care
Willingness to adapt to value-based care setting
Superior patient/customer service skills
Basic computer skills including email and EMR
Preferred Qualifications
Board Certification or Eligible in Family Medicine, Internal Medicine or Geriatric Medicine
Active and unrestricted DEA license
Medicare Provider Number
Medicaid Provider Number
Two to five years applicable experience
Experience managing Medicare Advantage panel
Knowledge of Medicare guidelines
Bilingual
Knowledge of HEDIS quality indicators
