Job Description

The Case Manager at The Jewish Board plays a crucial role in supporting adults with serious mental illness. You will guide residents in creating personalized recovery goals and developing independent living skills, all while maintaining a compassionate, culturally competent approach.

Key Responsibilities

  • Carries a caseload and practices person-centered and collaborative service planning with residents
  • Ensures service plans are completed and signed by residents
  • Evaluates and assesses independent living skills
  • Supports residents toward medication management independence
  • Advocates on behalf of residents with other service providers
  • Completes regular and timely documentation of all resident interactions
  • Links residents to community-based treatment and collaborates with other service providers
  • Participates in intake, assessments, discharge, and follow-up conferences

Required Qualifications

  • Bachelor of Arts (BA) or Bachelor of Science (BS) degree
  • Experience working with individuals living with serious mental illness, substance use disorders, or trauma

Preferred Qualifications

  • At least 1-year experience in Human Services
