About The Position

• Coordinate and manage individualized care plans for patients across the continuum of care.
• Collaborate with interdisciplinary teams to support discharge planning and care transitions.
• Assess patient needs, resources, and eligibility for services and community programs.
• Ensure documentation and utilization reviews meet regulatory, payer, and hospital guidelines.

Job Requirements

• Active Registered Nurse (RN) license in the state of practice
• Minimum 2 years of recent RN experience; Case Management experience preferred
• BLS certification required
• Familiarity with EMR systems (EPIC preferred)
• Strong knowledge of discharge planning, utilization review, and community resources
